A good source of such Safety Moments that covers a wide variety of sectors is the Department of Energy's Operating Experience web site found at
https://opexshare.doe.gov

Their report are on a variety of topics, ranging from laboratory settings to industrial conditions. I like particularly that the audiences for these reports are organizational wide, from trades workers to researchers to management with many levels of granularity in the discussion. It is a resource well worth exploring.
