I have added a Safety Analysis and Risk Assessment flow chart to the bottom of the web page at the URL:

https://ravensbyre.net/safetyanalysis/safetyanalysis.html

Its value is that is constructed using mostly plain language guidance rather than the jargon of risk assessment. The intention is to make the process more transparent and logical to those unfamiliar with the lexicon used by safety analysts.
