The NFPA Journal published an interesting article, "Unsafe Science", on this topic and provides some historical background on other similar incidents.

It is because of such accidents that I initially got involved with David Finster and others to establish the CHED Safety Committee. I have been in contact with faculty in our Education College and have set up a meeting to explore how our departments can work together to incorporate concepts of safety awareness, hazard identification, and assessment into the education programs for their future science teachers. I am hopeful that our discussions will be productive and we will also be able to reach out to our local school district science departments.. Regardless of which fire code has been adopted by our respective states, or local fire departments, safe chemical handling practices will always help to prevent accidents.

I would like clarification. The article referenced below states that it is retroactive to all schools. Is that regardless of whether or not your state has adopted the 2015 version? If so, it would make it much easier for some of us to push this as standard practice.

"The new requirements in Chapter 12 of NFPA 45 are retroactive to all schools and apply to the performance of science demonstrations using hazardous materials. The requirements include instructor responsibilities, chemical handling and storage requirements, and controls for the performance of demonstrations. The requirements below are similar to the requirements that the CSB recommended last October."
