Good afternoon,

Notre Dame is currently looking at conducting risk assessments(physical hazards as well as chemical, radiological and biological)  of each of laboratories on campus.  Has anyone done this before and willing to share the matrix you used?

Whether or not you have done a formal risk assessment, have you developed a process or identification factor(s) that  ranks labs for an inspection schedule (i.e. Low risk - every two years, high risk =96every year, etc).  If so, would you be willing to share?  You can post to the list or to me privately.

Thanks,
Lisa
