, I am keeping a running tally of incidents/near misses etc in our teaching labs including what type of incident and what it involves. Is anybody else doing this and would you be willing to share your data with me?

In this regard, people may be interested to know that the DCHAS-sponsored group is making (slow) progress on an generic "lessons learned" form to collect this information (anonymously) with regard to academic lab incidents. We're close to a beta version of this form now; if anyone would like to review it and provide comments, it's at
