Can we have a near-miss/incident/accident reporting page which will rely more on personal inputs as opposed to formal reports?

We have a committee working on this idea. It turns out to be more complicated than I thought at the beginning, but I hope that we'll be able to have something available for people to use in this way in the first half of 2013.
