I agree with the suggestion to contact the Laboratory Safety Institute.  I have their Model Chemical Hygiene Plan and used it as a template.  I believe is costs about $100.  It refere nces High School labs also and I think provides a great format that most labs ca n use.  I have also attended several courses through LSI.  Dr. Kauf man and the employees at LSI have and provide a wealth of chemical safety information.

Has anyone at a university written a Chemical Hygiene plan recently?

Our Chemistry Department  is rewriting/updating o ur Chemical Hygiene plan.  Does anyone know of a good place to find information on the latest requirements for a Chemical Hygiene plan for a university?

Does anyone know of a template that can be used?
