I suggest contacting Jim Kaufman at Laboratory Safety Institute. They have a Chemical Hygiene Plan template available that may suit your needs.

Has anyone at a universi ty written a Chemical Hygiene plan recently?
Our Chemistry Department  
i s rewriting/updating our Chemical Hygiene plan.  Does anyone know of a good place to find information on the latest requirements for a Chemical Hyg iene plan for a university?
Does anyone know of a te mplate that can be used?
