We have students sign a safety pledge in each lab course each semester.  We
 keep these for one full semester after the course is completed.  When we h
ave any minor injury or incident, we keep a copy of the safety pledge with 
the incident report. We're small and accidents are rare.

I have thought about doing an electronic safety quiz and pledge.  Does anyo
ne do it that way?
