Subject: CHAS Programming Survey - IMPORTANT
CHAS members -


As one of the CHAS program chairs, I am always trying to improve the
quality of the technical program at National meetings.  It is extremely
important that programming meets the needs and desires of members.  With
this in mind, please take a moment to complete the brief survey below.
All you need to do is reply to me personally (NOT the list), mark your
answers to the questions below and send the reply to
jim**At_Symbol_Here**chemical-safety.com.  All responses will be kept confidential unless
otherwise indicated.  If you wish to reply anonymously, please print
this out and send to the address below.  I will tabulate the answers and
prepare a report that I will distribute back to the group and present at
the executive committee meeting in Anaheim.

later than COB 3/5/04.  Your responses are important and we will act on
